Pay Range: $19.00 - $24.00 an hour

What You'll Do As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team - not just a support role, but a trusted presence for patients and families. Your responsibilities include:
- Providing personal care with dignity, comfort, and respect at the center of everything
- Attending to patient needs and requests promptly and compassionately
- Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
- Observing patients and reporting changes to the care team accurately and in a timely manner
- Communicating with patients and families in a positive, ethical, and confidential manner
- Maintaining thorough documentation in line with organizational standards
- Upholding the hospice philosophy with professionalism and genuine compassion

What You Bring - Active CNA License or Certification in the state of employment
- Possess and maintains current CPR certification
- One (1) + year Hospice, home health or related organization experience preferred
- Comfort and sensitivity working with terminally ill patients and their families
- A calm, dependable presence under emotionally demanding circumstances
- Strong attention to detail in observation and documentation
- A valid driver's license and willingness to travel locally for patient visits
